A typical shift might include helping with personal care and other expected caregiver tasks, but we also want to provide meaningful activities, personalized meal planning, interaction, and community connection. This will likely be different for each client we take on, and we want our caregivers to play an integral role in assessing and supporting what their clients need and want.

Requirements

  • Ability to communicate with clients and to remain calm under stress

  • Ability to read and write in English

  • A Certified Home Care Aide or state Certified Nursing Assistant certification

  • Ability to pass a background check (may require fingerprinting)

  • Knowledge of personal care techniques and health monitoring

  • Excellent communication and interpersonal skills

  • Availability to work at least 30 hours per week during the schedule of your choice

  • Reliable transportation and a valid driver's license and car insurance

  • Ability to follow oral and written directions and complete work as assigned

  • Ability to maintain confidentiality about client services and personal information

  • Maintain current state driver’s license and dependable insured automobile

  • Health status good and ability to lift, bend, squat 35#

  • Accurately document Certified Caregiver services provided

  • Present a neat appearance and positive work attitude

Job Description

1. Demonstrates support for the agency mission statement to promote quality, efficient, comprehensive, and effective client services.
2. Participates in and supports the agency’s performance improvement activities
3. Provides Certified Caregiver services to clients as identified by the agency in the plan of services per agency policies, procedures, and Certified Caregiver skill level.
4. Prepares and maintains accurate and timely client notes, documenting interventions, client pain score, outcomes, and client response per agency policy.
5. Assists with personal care activities (ADLs) and IADLs allowing client independence to tolerance as indicated in the service plan including:
a. Personal care and hygiene
b. Universal precautions
c. Locomotion/ambulation/fall prevention oversight
d. Nutritional support, meal preparation, feeding assistance
e. Bed mobility and positioning, range of motion exercises
f. Toileting
g. Continence care
h. Dressing and undressing
i. Bathing
j. Foot care
k. Skin care
l. Medication reminder assistance
m. Light housekeeping including dusting, vacuuming, linen changes
n. Laundry of client’s clothing and bedding
o. Basic technology and phone assistance
6. Social and emotional support
7. Meal Planning and Preparation according to client's desires and dietary restrictions
8. Meaningful Activity planning and participation
9. Reports client condition changes to supervision.
10. Appropriately seeks supervisory assistance as necessary to effectively perform duties.
11. Maintains confidentiality of client, employee, and agency operations.
12. Meets productivity requirements established for Certified Caregiver.
13. Participates in ongoing training as required.
14. Responsible for proper use and care of equipment.
15. Practices safety, universal precautions, and infection control measures.
16. Maintains a neat appearance and positive attitude with clients, family, and personnel.
17. Performs other duties as qualified and requested.
